Something I've wondered about, and the conversation in the Heavy Lift thread brought that into sharp contrast - Nasa always talks about man-rating a rocket.

Question - is there exactly a step-by-step process/prochedure that has been specified in Nasa literature somewhere, or not?

Considering Challenger and Columbia, it would be fair to characterize this as a “systematic but flawed safety reviewâ€
